RLM: Creating Duplicate Lines Even With Multiple Matching Attributes (Doc ID 1429771.1)

Symptoms

Processing inbound 830 with firm demand lines. For some lines with a Request Date as the current date, and matching attributes of PO Number, PO Release Number and Request Date/Time, some lines are getting duplicated.
In some cases it appears these original lines that are being duplicated are in some state of the shipping process.
Out of 39 original lines with a request date of 20-FEB-2012, 4 were duplicated. What is causing this erratic duplication?
