R12: Upgrade Patch 7303030 Fails with "ORA-01017: invalid username/password; logon denied"

Symptoms

Customer is on release 12.0.x of the E-Business suite. When applying Release 12.1.1 upgrade patch 7303030, error "ORA-01017: invalid username/password" is hit as follows:
