Some Users Get: Oracle error 1403 ORA-01403 ORA-06512 FND_SESSION_MANAGEMENT.CHECK_SESSION (Doc ID 1438938.1)

Symptoms

Most IE9 Users are having problems logging in to the new R12.1.3 instance on Windows 7 64-bit.
The issue does not occur for other IE browsers, Chrome, or Firefox 9.0.1.
They can connect to their 11i instance using IE 9 32-bit on Windows 7 64-bit.
Some users running IE 9 and CAN access the system.
This points to a configuration issue with the desktops.


STEPS TO REPRODUCE:
After entering my username and password, get the error shown below:

<PRE>Oracle error 1403: java.sql.SQLException: ORA-01403: no data found ORA-06512: at line 1 has been detected in FND_SESSION_MANAGEMENT.CHECK_SESSION. Your session is no longer valid.

 
No errors in the db alert log.

Another oddity is that sometimes the user can login, but then as soon as they log out then they cannot login again. It's like after a while, what ever is keeping the user from logging in is released and the user can login, but then once users log out then the users cannot login again until after a period of time. Tried bouncing the Applications and Database tiers and this didn't help.

Changes

Upgrade to R12.1.3 instance on Windows 7 64-bit and users are using IE9.
