Symptoms
On : 12.1.3 version, Subledger Accounting
When attempting to run the Create Accounting -Cost Management,
the following error occurs.
ERROR

XLA_AE_HEADER_PKG.InsertHeaders. ORA-24381: error(s) in array DML
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Connect to Cost Management - SLA Responsibility.
2. Go to Requests Form and Run Create Accounting Cost Management
3. Check the Log file and the Error is generated.
