Symptoms

On : 12.1.3 version, Service Request Tab

ACTUAL BEHAVIOR
---------------
In eBusiness Center form Service Request tab the Attributes button disappears as soon as information is entered in the eBusiness Center header.

EXPECTED BEHAVIOR
-----------------------
Expected the Attribute button to not show up as the button does not work also.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Open eBusiness Center form
2. Navigate to Service Request tab and notice that Attribute button is displayed.
3. Enter customer information in eBusiness Center header and the Attributes button disappears from the Service Request tab.
