Error FRM-92101 When Doing A Blind Query On Incident Address LOV (Doc ID 1448362.1)

On : 12.1.3 version, Service Request Form

In the Service Request form header, when attempting to change the incident address by searching the incident address field using a blind query (%), the following error occurs.


There is an FRM-92101 and the form session is closed.
The Details of the FRM-92101 error indicates:
  Java Exception: Forms session <7> aborted: unable to
  communicate with runtime process.

Sometimes the LOV shows the following errors:

ORA-01403: no data found

FRM-40502: ORACLE error: unable to read list of values.

The issue can be reproduced at will with the following steps:
1. Go to Service Request form
2. Query a customer
3. In the Incident Address region open the LOV and perform a query using %
4. Reported error occurs
