Symptoms

Credit Memo creation appears to progress normally when applying Credit  Balance, acknowledgment  message is received:
APP-AR-11641: The balance amount of the transaction has been credited. Please save your changes and complete the credit memo

But when you proceed to save, the following error is raised:
ERROR: An unexpected error has occurred. Please contact your system administrator.

FND Debug log shows :
