Symptoms

After applying <Patch 13985008> - 12.1.3:BUG FIXES FOR CS: APR'12 PATCH - REPLACEMENT (including pre-reqs), updating a Task in the SR form throws an error.

Error:

FRM-92101: There was a failure in the Forms Server during startup. This could happen due to invalid configuration.
Please look into the web-server log file for details
