Debrief Form Error: 'FRM-92101: There was a failure in the form server during startup' (Doc ID 1464701.1)

When attempting to save a record in the Debrief form, the following error occurs:

FRM-92101: There was a failure in the Forms Server during startup.This could happen due to invalid configuration.
Please look into the web-server log file for details.

Java Exception Forms session aborted: unable to communicate with runtime process.

The issue can be reproduced at will with the following steps:
1. Open Service Request form.

2. Fill in the details and save the service request.

3. Create a task for this service request.

4. Go to Debrief, enter details and save.
