In R12.1.3 instance, the following error is received while adding the province code to the Geography Hierarchy for Canada:

The issue only happens in one instance and not in the other.

In the instance where the issue occurs, the geography for Canada has been setup successfully initially but there were some issues as users had chosen the wrong effective date for the provinces that we added. Subsequently users went ahead and deleted the provinces (from front end). After that whenever users are trying to add the province codes again, they are facing the issue on pressing the add 5 rows button. In the other instance where there is no issue, users just went ahead and setup the provinces for the first time.

The issue remains after bouncing the apache and clear cache.

Steps to reproduce:

-Trading Community Manager Responsibility, N: Trading Community > Administration > Geography Hierarchy
-Select Country Code 'CA' and click ‘Go’
-Click Structure icon and add ‘Province’
-Add ‘Province’ and click Apply
-Click View Details icon.
-Click the button ‘Add 5 Rows’. The error appears.
