AGIS: Approval Notification Not Appearing For Recipient

Information in this document applies to any platform.
Transaction is submitted by Initiator, however the recipient is not receiving the notification. Transaction type requires recipient approval and all transactions are stuck due to this.
AGIS Security setup is as follows:
Transaction type requires receiver approval
BSV 101 is assigned to User 1 which has access to transaction and send approval notification check box is ON
BSV 125 is assgined to User 2 which has access to transaction and send approval notification check box is OFF
BSV 125 is assgined to User 3 which has access to transaction and send approval notification check box is ON
Transaction was initiated by User 1 between BSV 101 and 125 and approval notification was sent to User 3 and not User 2.
