R12/CE: Upgrade from 11: Form Validation Failures A Value Must be Entered for Tolerance Amount etc (Doc ID 1472695.1)

Symptoms

After upgraded from 11i to R12, the Tolerance Amount and the Tolerance Percentage in the Account Controls were input null.
These columns must be input values. (The default values are 0)
When you try to Update Bank Account: Account Control: Errors :
  
Form validation failures:
  A value must be entered for "Tolerance Amount".
  A value must be entered for "Tolerance Percentage".
  A value must be entered for "Tolerance Amount".
  A value must be entered for "Tolerance Percentage".
  A value must be entered for "Tolerance Amount".
  A value must be entered for "Tolerance Percentage".
  A value must be entered for "Tolerance Amount".
  A value must be entered for "Tolerance Percentage".
  
Step to reproduce

This is using Cash Management Super User:
Setup>Banks>Bank Accounts>Update Bank Account>Account Controls - Values must be entered for the Tolerance fields

Impact on Business

Unable to view bank account setup from 11i to R12 without receiving an error.
