Notes Document Catetory Was 'replaced' With Miscellaneous, Cannot See Attachments On Service Request (Doc ID 1478416.1)

Goal

After upgrading to R12.1.3 it is not possible to view all the attachments that were added to a Service Request.  The categories that they were included in also is no longer available as a category option in the Attachments form.
Why is that?
