Error: 'Database Operation Failed' When Attempting to Approve Service Contracts

Symptoms
When submitting a contract for approval the contract goes into 'Pending Internal Approval' status, but an approval notification is not received. An error notification is generated with the error below.
This issue occurs when Auto Approve and Auto Sign of the contract workflow is setup.
Error:
ORA-20002 3133 Activity instance 'REVIEW_AND_APPROVE

Steps to Reproduce:
1. (R) Service Contract Manager
2. (N) Launch Pad
3. Create a new contract and submit for approval.
