Orphaned Attachment Records Upon Dropped Client Connections

Symptoms
After cleaning up orphaned records, as per Doc ID 963222.1 / Patch 9004099, a few months later the instance has new orphaned records. The orphaned records seem to be associated with dropped/lost client connection while uploading attachments.
Internal ATG Support testing, while simulating a dropped client connection, shows that FND_LOBS contains attachment records without an expiration date and without any relationship to the FND_DOCUMENTS table.
