Service Desk (CS HTML) - Error While Creating SR (Doc ID 1493813.1)

From the Agent Dashboard when trying to Create a new Service Request using the Existing Service Identifier and the Search Key = Employee Name
The Employee is not populated and after clicking on the Go button the following error appears:

"You have encountered an unexpected error. Please contact the System Administrator for assistance.
Click here for exception details."
