TeleService: "FRM-40350: Query Caused No Records To Be Retrieved" When Creating Service Request

When attempting to create a Service Request for an item that already has SR's open against it, a duplicate pop up window appears.
After hitting Continue Creation, enter all the SR information and save, a second duplicate pop up appears but with a FRM-40350: Query caused no records to be retrieved in the lower left corner.
Click Not a Duplicate.
A SR Number is created but when trying to re-query it, it does not exist.
FRM-40401: No changes to save when saving the SR.
In the Find Service Request form, the following error appears when trying to query the SR that was created:
FRM-40212: Invalid value for field INCIDENT_NUMBER.
1. Create a SR for an item that already has SR's open against it.
2. Receive the First Duplicate pop up > hit Continue Creation > enter all SR information > Save >
3. Receive 2nd Duplicate pop up but with FRM-40350: Query caused no records to be retrieved in lower left corner > click Not a Duplicate > SR # IS created
4. Try to use the Find SR form to query the SR, but then the form says it does not exist!!
