Service: Unable to Set Profile Option 'Service: Default Service Request Type'

Symptoms
* Issue # 1:
When trying to query the profile option Service: Default Service Request Type to see its values:
Receive an error message, and the profile option never displays in the form.
* Issue # 2:
If the profile option is displayed, it is not possible to select a value.
Error:
Profiles couldn't determine the valid values for profile INC_DEFAULT_INCIDENT_TYPE.
This problem is usually due to error in the SQL validation for that profile.
Use the 'Define Profiles' form to fix the SQL Validation for the INC_DEFAULT_INCIDENT_TYPE profile.
This error may appear immediately following and Oracle error, which give more information about what was wrong with the SQL Validation.

(N) Others > Profile Option Values
- Query profile option 'Service: Default Service Request Type'
