Unexpected Error when Creating a Performance Management Plan (PMP) and Adding Attachment NullPointerException (Doc ID 1502663.1)

Symptoms

On : 12.1.3 version, Appraisals

When attempting to Create a Performance management plan (PMP), there's an attachment button. While attaching a document, on when pressing the apply button
the following error occurs.

The issue can be reproduced at will with the following steps:
1. HR Professional v4.0 > Performance Management >
2. Performance management plan link > Desktop files (GO) > attach the document
