Symptoms
After compiling CUSTOM Forms from R11 to R12, the fields got displaced or missing.
Once on Apps, the fields in the form looks displaced, some of them are superposed upon the others, and there are additional fields too, that doesn't appear on the forms builder screen.
Also tried to create a simple Form from scratch, using the new TEMPLATE.fmb from R12, but got the same results -- misplaced labels and fields.
After reviewing the process in detail, we noticed that the following error appears when compiling the CUSTOM Form through the frmcmp_batch.sh utility:
