AGIS: Email Notification Is Not Working (Doc ID 1519821.1)

Symptoms

AGIS email workflow notifications do not work.
Workflow notifications are being received in the worklist, for errors as expected. However, the email notification is not working, even after configuring the workflow mailer.
The notifications are received in the worklist, but there are no email notifications.
  
EXPECTED BEHAVIOR

It is expected that the email notifications would be sent.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Setup AGIS
2. Configure workflow mailer
3. Set the Notification Style in Global Preferences to "HTML mail with attachments"
