1099 Electronic Media Was Rejected By IRS

2012 patch for 1099 reporting has been applied (Patch 13461067)
1099 Electronic Media tests to the IRS are being rejected with the following.

1 'A' record(s) has an incorrect/invalid type of return and/or amount code(s) in positions 26-43. Refer to the current Publication 1220, Part C, Section 5.07, for information concerning this error.
Record sequence number 2 is the first known record containing this error.

161 'B' record(s) is coded for Combined Federal/State Filing Program; however, the type of return indicated in the 'A' record is not part of the program. Only Forms 1099-B, 1099-DIV, 1099-G, 1099-INT, 1099-K, 1099-MISC, 1099-OID, 1099-PATR, 1099-R and 5498 are part of the program. In addition to removing the '1' from the 'A' record, also remove any corresponding codes in the 'B' record(s) and remove the 'K' record(s).
Record sequence number 5 is the first known record containing this error.
1099 patch for 2012 was applied, but the 1099 patch for 2011 (the previous year) was never applied.
