Concurrent Request Log Files Contain Old Record For Other Requests And Appended With New Record (Doc ID 1525353.1)

Symptoms

On Oracle Applications 12.1.3 version, Request Processing,
the "Active Users" concurrent request log contains information for other requests which has been executed before and appended with new data as below:

Expected Behavior:
The concurrent log file should contain only new data.

The issue has the following business impact:
It may make confusion when reviewing log files.

Changes

 Database has been restored several times while file system remain unchanged.
