This can occur when an employee is updated or expired in the People(HR) window.
Expiring an employee, the assigned responsibilities will be physically deleted.
There has been cases when other changes for an employee, e.g. changing email, resulted to the same but it could not be replicated at will
The employee assigned to the FND user was expired from HR, in the People window
