TeleService Error: 'FRM-41826 Cannot Replace Group; Columns Don't Match LOV For field INSTANCE_NAME' On Service Request Tab (Doc ID 1534008.1)

Symptoms

On 12.1.3, Service Request tab on Contact Center form, it is found that the Instance Name field LOV does not work and gives an error:

  "FRM-41826 Cannot Replace Group; columns don't match LOV"

Steps:

- Add the 'Instance Name' field with Folder tools.

- Enter the  customer name for SR, then use the list of values for the Instance Name field.

- Error is received.
