TeleService: Workflow Is Calling Twice From Service Request Form - Tasks Tab (Doc ID 1534974.1)

Symptoms

In Service Request - Tasks tab:
When pressing on Launch Workflow button after making changes, including adding comments to a Descriptive Flexfield (DFF), the Workflow is launched twice.

Steps:

The issue can be reproduced at will in 12.1.3 environments.

1. Open the Create Service Request form
2. Create a Service Request and navigate to the Tasks Tab
3. Select a task type and save the Task
4. Update the DFF and save.
5. Return to the Tasks tab and click on the Launch Workflow button
=> There are 2 workflows in the Workflow Status Monitor for the same Task.

Note that there is a message 2 rows updated.

Changes

Upgrade to Release 12.1.3.
This did not happen in 12.0.6.
