"Invalid Payment Instrument Assignment" while Placing Credit Card Orders Through iStore After Upgrade from 11i

On Prod (Upgraded from 11i) : 12.1.3 version, Order Submit

When attempting to place order with credit card
the following error occurs.

Transaction completed successfully
Invalid payment instrument assignment.
ERROR_RETURNED (API=IBY_FNDCPT_TRXN_PUB.Create_Transaction_Extension )

The issue can be reproduced at will with the following steps:
1. Login to iStore as an user that has credit card in 11i (prior to R12 upgrade)
2. Add item to cart
3. Checkout using existing credit card
4. Place order and receive error

The issue has the following business impact:
Due to this issue, users cannot place an order
