Learner Receives Error Notification "The changes were not applied because The primary key specified is invalid..."

After a learner has requested enrollment in a class, and the manager approves it, sometimes the request is not approved and the learner gets an error notification with the following error message:
ERROR ----------------------- The changes were not applied because The primary key specified is invalid Cause: The primary key values specified are invalid and do not exist in the schema. Action: Check the primary key values before attempting to carry out the operation again'.
STEPS ----------------------- The issue can be reproduced at will with the following steps: 1. Learner requests class enrollment 2. Approver approves Learner gets notification with the error message
