TeleService: Clicking On Extended Attributes Button In SR Form Hangs (Doc ID 1539388.1)

When click in the SR Form Workbench Attributes button, the form hangs.

The form should display the window to enter the Extended Attribute values.

The issue can be reproduced at will with the following steps:
1. Define a new Code in Lookup Type CUG_SR_TYPE_ATTRIBUTES
2. Map SR Type to the newly defined code and assign a List Name
3. Enter a new SR and when click the Attributes button the form hangs
