E-Business Center Form Crashes When Trying to Save an Account under the Account tab. (Doc ID 1542320.1)

Symptoms

On : 12.1.3 version,

When creating an account for an organization in ebusiness center form under Account tab and Save, got an error.
 
ERROR
-----------------------
FRM-92101: There was failure in Forms server during startup this could happen due to invalid configuration.

The issue can be reproduced at will with the following steps:
1. Login as Telesales agent to ebc
2. create an organization or query an existing one
3. under Account tab, create a new Account and Save , got the error
