Query a Customer Account after upgrade or Customer created through Customers Interface gives Error Flexfield Segment Context Value Does Not Exist In The Value Set $FLEX$.DF_CONTEXTS

On : 12.1.3 version, Customers Module

After upgrade from 11.5.10 to 12.1.3, while opening Customers form, the following error occurs.

ID <xyz> for the flexfield segment Context Value does not exist in the value set $FLEX$.DF_CONTEXTS.

Same Error was also seen with Customer Interface

Create a new customer using Customer Interface.
Query up a customer on the UI
Click on "Details" Icon under "Create Account" Button.
Error will appear.
