On :  12.1.3 version, Receiving Functions
An upgrade has been performed from R11.5.10.2 to R12.1.3.

While creating a receipt based on the purchase order the following error occurs

RVTTH-010: Subroutine rvtthins() returned error
Cause: Subroutine rvtthins() returned an internal error.
Action: Note this error number and the actions you are trying to perform. Contact your system administrator for assi
RVTTH-192.rvtthins: Subroutine ORA-00001: unique constraint (PO.RCV_TRANSACTIONS_U1) violated returned error
Cause: Subroutine ORA-00001: unique constraint (PO.RCV_TRANSACTIONS_U1) violated returned an internal error.

Inventory debug log file shows

The issue can be reproduced at will with the following steps:
1. Receiving > Receipts RCVRCERC form
2. Query a PO .
3. Enter receiving information and save.
