Not able to create an Invoice or Credit Memo using custom or standard responsibility (Receivables Manager), the following error occurs.

FRM-40735: ON INSERT trigger raised unhandled exception ORA-00911

Steps to Reproduce:
Responsibility:  Receivables
Navigation:  Transactions > Transactions
Create a Transaction
Click Line Items
Button is greyed out > Error occurs
 
The Transaction Type List of Values (LOV) shows the same Transaction Type multiple times (two times or more).
Issue is happening for all Operating Units. 

The database is showing that the Transaction is getting saved, but do not appear when queried from the form.
Find that the  CUST_TRX_TYPE_ID stamped in table RA_CUSTOMER_TRX_ALL is wrong.
