ACTUAL BEHAVIOR
---------------
In Customer Support Specialist, during SR creation, in the "Contact Information" section the "Contact By" LOV doesn't provide any way to differentiate between fax or phone numbers.

EXPECTED BEHAVIOR
-----------------------
There should be a way to differentiate between fax and phone number.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. In Customer Support Specialist start a new SR creation.
2. In the Contact Information section add a contact and open the Contact By LOV
3. There is no way to differentiate between fax and phone numbers.
