Symptoms

Attempting to add Items to a Sales Order, after applying <Patch 15877122>, and receive the following error:

An unexpected error has occurred. Please contact your system administrator.

Review of the FND log file shows the following:

ZX.PLSQL.ZX_SRVC_TYP_PKG.CALCULATE_TAX ORA-06508: PL/SQL: could not find program unit being called
