R12: Getting FRM-40654 Error While Creating or Updating Requisitions

When attempting to save a requisition the following error occurs.
FRM-40654: Record Has Been Updated. Requery Block To See Change
Steps to reproduce
1) Open the Requisition entry form.
2) Click on Save (Header record saved first).
3) Now enter the line level details and click on save.
4) Get the FRM error.
