E-Record In Service Request Approval Notification Does not Contain the Attached Document (Doc ID 1567159.1)

Symptoms

When an e-record is created from a Service Request, the attachment is not accessible.

The E-Record in the Service Request Approval does not contain the Attached Document.
In the E-Record the attached file does not exist although the message "Review the attached e-record document, eRecord_9001.pdf" does appear.
