Assignment Change Reason Not Being Saved To The Database From SSHR Hire Action In 12.1.3

On :  12.1.3 version in Production , Hire, CWK Placement, Appl Hire

After performing an Applicant Hire and Rehire, entering all details including Assignment Information including Change Reason,
when reviewing the person information after rehire, it is found that the Assignment Change Reason is no longer populated

Expect all the details entered for the Hire and Rehire to exist upon re-query.

The issue can be reproduced at will with the following steps:
1. Manager Self Service
2. Applicant Hire and Rehire
3. Enter all required details, including the Assignment Change Reason and after save, it is no longer populated upon requery.

The issue has the following business impact:
The only workaround for this issue is to have users enter the change reason in the Professional User Interface (PUI) forms after the action has completed. This adds to the workload of users and creates a possibility that the reason may be forgotten. We are currently in our CRP-2 and need this resolution so testing can complete with successful acceptance by HR Users.
