Symptoms
On : 12.1.3, when applying patch 16982194:R12.CSM.B it fails with an error
ATTENTION: All workers either have failed or are waiting:
FAILED: file csm5vers.sql on worker 1.
FAILED: file csm5stpi.sql on worker 2.
ATTENTION: Please fix the above failed worker(s) so the manager can continue.

ORA-06508: PL/SQL: could not find program unit being called:
"APPS.CSM_HTML5_PKG"
