TeleService: Errors While Opening Main Service Request and SR Types Forms - FRM-40735 ORA-06508 ORA-01403 (Doc ID 1582269.1)

Symptoms

There have been two reported situations where this type of error has been generated:

 

1 - After applying cumulative Service patches, Service Request forms are giving multiple errors.

[ A ] In main Service Request form - Subject tab:

 * FRM-40735: CLOSE_WINDOW trigger raised unhandled exception ORA-06508.
 * An Unexpected Error -6508 has occurred. An alert has been sent to the system administrator.
   ORA-01403: no data found

[ B ] In Service Request Types - setup form:

 * An Unexpected Error -6502 has occurred. An alert has been sent to the system administrator.
   ORA-01403: no data found

Steps:

As per Doc ID 1326683.1 applied CS (Service) and CSC (Customer Care) patches.

(R) Service
(N) Service Requests > Create Service Requests
(N) Setup > Service Requests > Service Request Types

 

2 - After upgrading to R12

When attempting to close the Service Request form after upgrading to R12, the following error occurs:

FRM-40735: CLOSE_WINDOW trigger raised unhandled exception ORA-06508

Changes

 Applied Service cumulative patch.
