On : 12.1.3 version, Update Service Request

ACTUAL BEHAVIOR
------------------------

When creating a new contact for a service request on CS HTML interface, the user is forced to enter a new address for the contact.

It is not possible to choose one of the existing customer addresses for the contact.

This causes duplicate addresses to be created.
