Symptoms

When attempting to close the Service Request form after upgrading to R12, the following error occurs.
FRM-40735: CLOSE_WINDOW trigger raised unhandled exception ORA-06508 error


Steps To Reproduce:
The issue can be reproduced at will with the following steps:
1. Navigate to the Customer Support responsibility
2. Open the Create Case (Create Service Request) form.
3. Use the x at the top of the form to close and the error appears.
