Clear Record Error Message Appears Twice on Service Request Form (Doc ID 1588916.1)

Symptoms

On : 12.1.2 version, Service Request Form

When attempting to clear the service request form the following error occurs twice.

Error

Your data is not saved yet. If you choose to proceed, your data will be lost.
Cancel <> Proceed


Steps to Replicate

The issue can be reproduced at will with the following steps:
1. Open service request from
2. Enter any details.
3. Try to clear the data, the error message appears twice.

This is not expected.
