After Upgrade - New Ship To Location Error: Item Orderability Rules prevents the Item x belonging to Category x from being ordered

Symptoms
On : 12.1.2 version, Install / Upgrade
Last week business upgraded to R12.1.3 from R12.1.3.
After upgrade, we are facing issue that if sales order is created for any new ship to customer, system throws an error message related to item orderability(Screenshot attached).
We have checked the item orderablity setup related to SHIP TO & ITEM is everything fine.(screenshot attached)
Upgraded to 12.1.3 and added new ship-to and then new orders. Was able to book OK.
However, found later when adding a new ship-to and then new orders, getting error and not able to save the line.
ERROR

Item Orderability Rules prevents the Item x0001 belonging to Category from being ordered.
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. BILL TO Customer : Chem Nut (already existing customer created before upgrade)
2. SHIP TO : Customer1 (Address: GA new address)-> Created yesterday by business(after upgrade)
3. Order: 11111 with item: x0001 is created with above BILL TO and SHIP TO,system throws an error message while saving the order line (i.e Item Orderability Rules prevents the Item x0001..)
4. ITEM ORDERABILITY setup is fine (region:GA is enabled for the item: x0001)
If order is created for the item: x0001 with OLD SHIP TO (i.e created before upgrade), system is not throwing any error message.
After upgrade any order with new ship to address, we received Item Orderability rule prevent issue.
But Item Orderability setup is correct and no change in the Region "GA"
BUSINESS IMPACT

We can't ship NEW orders today due to this issue.
