BI Publisher Template Is Not Invoked When Using Copied Request Set

Symptoms
In EBS 12.1.3 (R12.FND.B.3)
1. A request set containing a program setup for BI Publisher processing, runs OK to generate the BI Publisher concurrent output.
2. After having used the Request Set Copy function (selecting this same request set),
when running the copied request set the XML (BI) Publisher template does not get invoked.
3a. The Submit Request, FNDRSRUN.fmb, is at version 120.44.12010000.43 (this is the version that allows BI Publisher requests to run in a request set).
FNDRSRUN has been updated to the latest version 120.44.12010000.82 but the behavior is the same.
3b. The Request Set form, FNDRSSET.fmb, is version 120.4.12010000.3 (delivered by the basic R12.ATG_PF.B.delta.3 patchset).
