Problems With Formatting, Parameters & Line Graphs In BI Publisher

In a new installation of BI Publisher Enterprise (upgraded from, after uploaded the rpd file through Enterprise Manager and brought the catalog across from the archive / unarchive process. Now  seeing the following problems in BI Publisher:

1. The Microsoft format mask on a number field #.# results in the field showing 1,2,3,4,5,6 instead of 123,456.

2. Number fields that contain a decimal point do not display. If the number is truncated to a whole number in the query, the number displays. See screen shot 2, where for example the average member spend control shows a truncated decimal value, and the average spend per visit shows the blanked out fields.

3. When parameter values are selected at runtime, all the parameters blank out, and sometimes the whole browser page shows as dark blue.

4. The line graph sorts alphabetically even when the sort has been removed. It can be removed in the Designer, but when one runs the report it is reapplied.

No errors are displayed.
E-Business Suite is not used.
