On : 12.1.3 version, Service Request Form

ACTUAL BEHAVIOR
---------------
The Updated Attribute values Note is not created.

When the Extended attributes for an existing service request are updated, the note recording the old and new attribute values is not being created.

EXPECTED BEHAVIOR
-----------------------
After updating extended attributes, a new Note should be created with the old and new attribute values.

Changes

 patch 16710952:R12.CS.B and/or 16712956:R12.CS.B  and /or 16943626:R12.CS.B were applied
