Vacancy Returned From Amendment After Being Approved, User Open Worklist Notification Error (Doc ID 1599801.1)

On : 12.1 HRMS RUP5 version, Requisition and Vacancy

A vacancy gets created by the recruiter.
The vacancy then gets forwarded to the approver who approves the vacancy.
The recruiter then goes in and makes an update to the vacancy and sends it off for approval again. When the approver approves it, the recruiter opens the notification to check the Vacancy. The system gives the below error:

The changes were not applied because User-Defined Exception

The issue can be reproduced at will with the following steps:
1. Create a Vacancy
2. Submit for Approval
3. Approver approve the Vacancy
4. Recruiter edit the Vacancy
5. Vacancy get sent to the Approver to approve it again
6. Recruiter open the Notification Worklist
7. Error is shown

The issue has the following business impact:
Due to this issue, users cannot update the Vacancy the second time.
