ADOP Fails With Error "Unable to find the run and patch context files"

Symptoms
Running adop phase=prepare fails with the following:
RUN file system context file: /u01/oracle/VIS/fs2/inst/apps/<SID_hostname>/appl/admin/<SID_hostname>.xml
PATCH file system context file: /u01/oracle/VIS/fs2/inst/apps/<SID_hostname>/appl/admin/<SID_hostname>.xml
[WARNING] Warning: The specified run context file seems to be incorrect
[WARNING] If ADOP can determine the run context file itself, ADOP will proceed, otherwise will error
[UNEXPECTED]Error: Unable to find the run and patch context files. Please pass the correct context files
[UNEXPECTED]Error while sourcing and validating the required ENV setup
Log file: /u01/oracle/VIS/fs_ne/EBSapps/log/adop/adop_20131111_130233.log
