Forms are not opening, because of incorrect values in opmnctl file

Unable to open forms after upgrading from 11.5.10 to 12.1.3. Solution provided was to re-deploy the .ear file.
The error below occurs while attempting to deploy the .ear file:

Upgraded from 220.127.116.11 to 12.1.3, and Database Upgrade from 10.2.0.3 to 18.104.22.168.
